如何辦理 State ID ? (by Yibi Huang)

為什麼要辦 State ID?

在美國，用信用卡買東西的時候、辦電話的時候、在銀行開戶的時候…凡是需要出示 Photo ID 的地方，駕照和 State ID 是最容易被接受的 Photo ID。有時候護照或學生證也可以代替，但不一定到處行得通，有些地方甚至指名只接受駕照或是 State ID。有些地方需要出示兩個 Photo ID 時，也可以拿學生證或護照和 State ID 配合使用。所以新生們如果不打算考駕照，或短期內不打算考的話，最好在下雪之前，結伴去辦 State ID。

在哪裡辦 State ID?

辦 State ID 地點是在 Downtown 一棟稱為 State of Illinois Center 的建築物的地下室，地址是 100 W Randolph St.，在 Clark 和 Randolph St 的交口，電話是 312-814-6660。

辦 State ID 的流程大致如下：像銀行一樣先抽號碼牌，等候叫號，輪到時號碼燈上會顯示到幾號窗口辦理。辦理時要填一張表，詳細內容我不太記得了，記得有地址、身高、體重、血型，這些都是會顯示在 State ID 上的項目，他們會要求出示地址證明，此時就把電話費帳單及 Bank statement 給他們看。表格上有一格簽名欄，要好好的簽，不要太淡、也不要出格，因為這就是會印在 State ID 上的簽名。表格交還他們之前檢查一次是否每一項都填對了，尤其是姓名是否都拼對了。他們還會問如果你發生意外的話，是否願意把器官捐出來? 因為 State ID 背面就是一張器官捐贈同意卡，這個選擇就由大家自己決定了。交手續費後，就可以到另一處等待照相，整理儀容，美美的照完相後，幾分鐘就可以領到 State ID 了。領到之後再確認一次 State ID 上的資料是否都正確，尤其是姓名，若是印錯了請他們馬上更正，否則等到回家才發現就要再跑一趟了。整個過程扣除等待的時間在半小時以內。

To change the battery, try the jewelry store on 53rd between Woodlawn and Dorchester. (Sorry I don't remember the exact location. It's at a corner.) They
asked for $5 (battery plus service) for my watch. Of course they sell watches, too. Looks like they are Korean.

Walmart in Ford City also changes batteries for customers. The service itself is free, i.e. you just have to pay for the battery (about $3.5). But they can't deal with a watch with a back-lid which needs a 6-pin clamp to open it. They can only open watches which have a small battery lid on the back.

If you carry U of C's insurance plan, you have to go to the student care center (SCC) first and they will refer you to a hospital only if necessary (SCC can deal with most problems). That also means you cannot go to any hospital by yourself otherwise the insurance won't cover. The number of the SCC is (773)702-4156. You have to make an appointment through a phone call before you go there. Prepare your SSN and tell them your symptoms through the call. Another problem is they might ask you to pay the summer health fee required for students who see a doctor in summer quarter and are not registered in summer.

在送醫前自我急救是很重要的，記得持續冰敷(有辦法的話，睡覺時繼續)。就寢時腳(受傷處)要高過心臟，愈高愈好，受傷的腳千萬不可以用力或是動到，不得已要走路時，也不可施力，最好有東西或人幫忙支撐。SCC的位置，從59th St和Drexel Ave.的入口進去，左邊就是了。最好找一個人陪你去，在醫院入口就去要求一張輪椅，給醫師看過後，可以免費要一付枴仗，如果沒有骨折或韌帶撕裂，花兩三個月復原也是正常的，這是我去年在這裡打籃球扭到的經驗，祝早日康復。
